Make sure you complete the questionnaire in full and answer all questions carefully (questions marked with an * are required), as any failure will delay the application process and may halt it completely. The Trustees’ decision about whether a form is correctly completed is final and no correspondence will be entered into. There are guidance notes at the bottom to help you fill in this form.

Notes for Guidance

  • Remember, your organisation’s address and activities must lie within the parishes of either St Mary’s Monken Hadley or Chipping Barnet Team Ministry.
  • Trustees meet four times a year, in January, April, July and October. To be considered in a particular meeting your application must be submitted by the end of the preceding month, that is: December, March, June or September.
